Registration
General Agreement: In consideration of the acceptance of this registration, and the holding of this seminar, and the opportunity to participate, I agree to hold Buddy’s Chance, LLC, Carolyn Vacchiano, Raising Canine, LLC, Susan Smith, and Roger Abrantes and assistants, the premises upon which the seminar is to be held and their employees and assistants, harmless from any claim for the loss or injury which may be alleged to have been caused directly, or indirectly to any person or thing by any act of dog or person while in or upon the premises or grounds or near any entrance thereto. I personally assume all responsibility and liability for any such claim. I hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expenses, including legal fees, by reason of the liability imposed by law upon any of the aforementioned parties for damage and expenses.
Photo/E-mail Release: I further agree to allow the aforementioned parties to use seminar videos and photos for educational, commercial, and promotional purposes and to contact the participant via e-mail for activities.
Cancellation: All cancellations must be made in writing and post marked or E-mailed no later than April 25, 2009 for a refund (less a $50.00 processing fee). Cancellations post marked or E-mailed after April 25, 2009 and no shows, will not receive a refund. Refunds requested after the no-refund date related to life crises including, but not limited to the death of an immediate family member may be granted (minus a reasonable processing fee) at the sole discretion of Raising Canine, LLC.

Dog Attendance Policy: With the exception of service dogs, dogs will not be permitted at this seminar. Please contact us if you need accommodations for yourself or a service dog.
